Documentation We Need to Evaluate Your Funding Request
The specific documentation we need differs based on the type of case in which you are involved. If the case is in litigation, we also need copies of the Complaint (or Petition), the defendant's Answer, and (if available) the Bill of Particulars.
Personal Injury Cases:
The police report (if you were involved in an auto accident, for example)
Incident report (if you were involved in a slip or fall incident, burn, dog bite, etc.)
Ambulance report (if available)
Emergency room records
MRI, X-Ray and operative reports (if available)
Proof of insurance policy (if available) on the part of the defendant
Medical or Legal Malpractice:
Medical reports and records or transcripts from the trial
Experts’ reports and credentials (if available)
Matrimonial (Divorce) Cases:
The agreements, e.g., separation, prenuptial, etc. (if any), entered between husband and wife
